Job Description
Summary: The MDS Nurse develops and completes the resident assessment process in accordance with the requirements of the federal and state regulations as well as the company policy and procedures under the direction of the MDS Coordinator. This position also assists the MDS Coordinator to ensure that the facility RAI process is in line with the prospective payment system guidelines and that a calendar is maintained to ensure appropriate reimbursement.
Qualifications: Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
Must be an LVN or RN in good standing and currently licensed by the State; MDS experience required. Must be able to work after hours and weekends as needed.
Education: Graduate of an accredited school of nursing.
Licenses/Certifications:
- Valid California RN or LVN license
- Valid CPR certification
Experience Required: Minimum of three years of clinical experience in a health care setting. Minimum of two years of Long-Term Care experience.
Essential Functions:
- Performs the development and completion of MDS in accordance with current federal and state rules, regulations and guidelines that govern the resident assessment, including the implementation of care plans.
- Assists in the development of a comprehensive resident assessment and care plan for each resident as a member of the facility's Interdisciplinary Care Plan Team.
- Updates the calendar for the Care Plan Team related to dates and time frames for assessment of new admissions, quarterly reviews and annual assessments along with any Medicare required assessments as requested by the MDS Coordinator.
- Facilitates and creates opportunities for family involvement and participation in the care planning process by conducting care plan meetings.
- Involve residents who are capable of participating in opportunities to develop their care plans and ensure their attendance at the care plan meeting if appropriate.
- Facilities with the involvement of appropriate health professionals i.e., therapists, or specialist physicians, needed to improve or maintain the resident’s functional abilities at the highest practical level.
- Ensures the validity of all MDS before electronic submission to the state and federal authorities.
- Conducts ongoing review of the MDS and care plans to ensure accuracy and consistency with the medical record.
- Completion and review of CNA Care Sheets for communication with direct care staff providing updates as required.
- Any other duties as assigned by the supervisor.
Knowledge/Skills/Abilities:
- Must possess strong clinical skills.
- The ability to communicate effectively with residents and family members.
- The ability to react decisively and quickly in emergency situations.
- Ability to prioritize and meet MDS assessment and submit by deadlines.
- Knowledge of training techniques for clinical staff.
- The ability to maintain confidentiality at all times.
- Knowledge of the Nurse Practice Act, state and federal regulations and facility policies and procedures regarding nursing standards and delivery of care.
