The Vineyards Healthcare Center is a skilled nursing facility dedicated to delivering high-quality care to its residents. We are seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our team.
Responsibilities:
- Conduct comprehensive assessments to evaluate patients' speech and language abilities, identifying areas of strength and need.
- Develop individualized treatment plans targeting specific goals, such as articulation therapy, language development, voice therapy, and cognitive training.
- Provide therapeutic interventions, collaborating with other healthcare professionals to coordinate patient care and monitor progress.
- Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
- Participate in team meetings and case conferences to discuss patients' progress and contribute to discharge planning.
Qualifications:
- New graduates or CFYs welcome; previous experience as SLP or ST is a plus.
- Valid state licensure as a Speech Language Pathologist and active CPR certification required.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) and good standing with regulatory agencies necessary.
- Strong knowledge of speech and language disorders, evaluation techniques, and evidence-based interventions essential.
- Excellent communication and interpersonal skills, with proficiency in computer skills and electronic medical records systems.
- Ability to adapt to changing priorities and thrive in a fast-paced environment.
