Job Title: Speech Language Pathologist (SLP)
We seek a dedicated and compassionate SLP to join our team at The Vineyards Healthcare Center.
Responsibilities:
- Conduct comprehensive assessments to evaluate patients' speech and language abilities.
- Develop and implement individualized treatment plans targeting specific speech, language, and swallowing goals.
- Provide therapeutic interventions, including articulation therapy, language development therapy, voice therapy, and cognitive training.
- Monitor and document patients' progress, making adjustments to treatment plans as necessary.
- Collaborate with healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.
- Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.
- Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.
Qualifications:
- New graduates and CF supervision are welcome.
- Previous experience as a Speech Language Pathologist or Speech Therapist is a plus.
- Experience in Skilled Nursing or Long-Term Care is a plus.
- Valid state licensure as a Speech Language Pathologist.
- Active CPR Certification.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) in good standing with all regulatory agencies and licensing boards.
- Interest in working with the geriatric population.
- Degree in Speech-Language Pathology from an accredited program.
- Strong knowledge of speech and language disorders, evaluation techniques, and evidence-based therapeutic interventions.
- Excellent communication and interpersonal skills.
- Proficiency in computer skills and electronic medical records systems.
- Strong organizational and time management skills.
