The Dietitian provides nutritional services to enhance patient well-being and outcomes, collaborating with an Inter-Disciplinary Team.
- Monitor patient data for timely interventions.
- Review lab work and recommend dietary adjustments.
- Counsel patients and families on diet adherence.
- Conduct comprehensive nutrition assessments within 30 days of admission.
- Participate in monthly patient care conferences and QAPI meetings.
- Document care plans and progress notes.
- Support customer service standards and maintain effective communication.
- Requires a Bachelors Degree, Registered Dietitian status, and state licensure.
- Preferred experience in dialysis.
