Job Description :
The Vistas at Bettendorf is currently seeking a Full-Time MDS Coordinator / Registered Nurse (RN) or Licensed Practical Nurse (LPN) who possesses excellent organizational, leadership, assessment and documentation skills. MDS experience in a long-term care setting Required, and a Nursing license in the state of Iowa.
Responsibilities :
Ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for our patients
Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs
Requirements / Qualifications :
Must possess a current, unencumbered, active state license to practice as an RN / LPN
1yr minimum MDS experience required
Experience in Skilled Nursing / Rehabilitation facilities preferred
You must be qualified, compassionate, and dedicated to a job well done
Utilize clinical documentation improvement strategies to ensure accurate coding and billing
Collaborate with the interdisciplinary team to develop individualized care plans
Monitor and update patient records in compliance with regulatory requirements
Provide education and training to staff on MDS completion and documentation best practices
Prior experience in HEDIS reporting and medical documentation
Knowledge of hospice care, case management, and utilization management practices
Understanding of Diagnosis Related Groups (DRG) and reimbursement methodologies
EducationMid Level (3-6 years)
