JOB RESPONSIBILITIES:
Assist with activities of daily living
Light housekeeping/laundry
Personal care assistance which includes bathing, personal hygiene
Medication pass
Provide companionship and basic emotional support
QUALIFICATIONS:
Must be a New York State Certified Home Health Aide (HHA) or Personal Care Aide (PCA)
Recent Physical, including a drug screen, PPD, and Rubella and Rubeola Titers
Ability to arrive timely with a self-driven and positive attitude
Ability to interact with the Residents with respect and dignity

HRIS Job Description:Summary: A person who carries out health acre tasks under the supervision of a Registered nurse and who also provides with personal hygiene, housekeeping and other related supportive tasks to a patient with health care needs in his/her home.
Job Duties:
Personal Care-assists with:
Bath (bed, bath, tub, shower)
Oral hygiene (mouth, denture care)
Care of hair (shampoo, dry and comb)
Care of nails
Skin care/lotion massage
Position change
Provide for elimination (bedpan, commode, toilet)
Assist with dressing
Take temperature and pulse.
Treatments-assist with:
Transfer and Ambulation
Test urine
Non-sterile dressings
Use of prosthesis
Active exercises
Use of special equipment
Takes vital signs as ordered
Intake and Output
Alcohol sponge
Assist self-medicating clients with medication
Homemaking- assist with:
Meal planning and preparation (prepare, serve, feed)
Assist with feeding
Linen change
Laundry
Light Housekeeping (making beds, dust and vacuum, tidy kitchen and bathroom, wash dishes after meals)
Grocery shopping, opening mail, banking and errands
The following functions are generally performed only after demonstration by the coordinating nurse:
Tub, bath or shower
Care of catheter drainage bag
Reinforce dressings and change simple, non-sterile dressings
Ace bandage/elastic stockings
Ice bag
Simple socks
Range of motion or prescribed exercises
Change or ostomy equipment
Use of rehabilitative devices such as walker, wheel-chair, crutches, cane
Special skin care
Application of binders and other supports
Oxygen equipment
Relearning of household skills
Measure intake and output
Prepare modified diets
Collect stool and urine specimens
Prepare formula
Breast care for nursing mother
Perform alcohol sponge baths
An aide is NOT allowed to perform these functions except as indicated in the Matrix entitled Permissible and Non-Permissible Activities: Home Heal Aide (HHA) services published by the New York State Department of Health on 08/19/92.
Enema
Colostomy or catheter irrigation
Gastric lavage or gavage
Administer medications
Change sterile dressings
Tube feedings
Give any medication-oral or injection
Apply any form of heat
Vaginal irrigation (douche)
Decubiti care
Make medical and/or nursing judgments
Any care not included in the nursing care plan
Documents care daily on all cases, reports to coordinating nurse any incidents or changes in condition of patient.
Participates in Performance Improvement Activities as indicated
Attends staff meetings and In-service presentations, as required.
Attends case conferences as indicated.
Communicates effectively with all those providing care.
Immediately notifies agency of any unforeseen circumstances or changes in the patient’s condition
Maintains patient safety and confidentiality
Observes and practices Standard Universal Precautions.
Qualifications and experience requirements:
Has successfully completed a basic home health aide training program approved by the New York State Department of Health and possesses written evidence of such completion, or
Has successfully passed an equivalent exam approved by the New York State Department of Health and possesses written evidence of such completion.
Three months of work experience as a Home Health Aide except trainees.
Function Abilities
Must be able to read twelve point or large type.
Must be able to lift, stoop and bend effectively so as to be able to perform the above listed job functions
Must be able to hear adequately with no more than an amplifier on the phone and speak in a manner understood by most person.
