- Assist clients in developing individualized transition plans , including housing, employment, healthcare, and recovery goals.
- Provide peer support services to help clients navigate early recovery challenges and maintain sobriety.
- Connect clients with community resources , including vocational programs, educational opportunities, social services, and healthcare providers.
- Work closely with case managers, therapists, and medical staff to ensure comprehensive care coordination .
- Conduct regular check-ins with clients to assess progress and address barriers to independent living.
- Facilitate life skills training , including budgeting, job readiness, and relapse prevention strategies.
- Assist clients in developing healthy coping mechanisms and support systems .
- Maintain accurate documentation of client progress, interventions, and referrals in compliance with facility and state regulations.
- Ensure adherence to Kentucky state requirements for CPSS and CSA services.
- Participate in team meetings, staff training, and program development initiatives .
- Active CPSS (Certified Peer Support Specialist) and CSA (Community Support Associate) license in Kentucky.
- Lived experience with substance use recovery and demonstrated ability to maintain long-term sobriety.
- Experience working in sober living, rehab, or mental health settings preferred.
- Strong knowledge of trauma-informed care, harm reduction, and recovery-oriented practices .
- Familiarity with Kentucky Medicaid services and community resource networks .
- Strong communication, organizational, and problem-solving skills .
- Ability to work independently and collaboratively within a multidisciplinary team .
- Valid Kentucky drivers license and reliable transportation for client appointments and outreach.
- CPR / BLS certification (or willingness to obtain).
- Experience with motivational interviewing, relapse prevention strategies, and evidence-based recovery approaches .
- Familiarity with 12-step programs, SMART Recovery, and peer-led support models .
- Experience using electronic health record (EHR) systems for documentation.
