The therapist functions as the primary clinician and is responsible for assessment, treatment planning, therapy / counseling, and education services for patients and families. Systematically and continually assesses the patient's mental condition in collaboration with the patient and family.Schedule 8 AM-4 : 30 PM Responsibilities General Description : The Department of Psychiatry at the University of Rochester invites applications for a full-time mental health therapist position to join the Strong Internal Medicine Behavioral Health team. The mental health therapist is an integral and embedded member of the primary care team at Strong Internal Medicine at URMC. Strong Internal Medicine is an integrated primary care clinic that has been accredited Level 3 Patient Centered Medical Home since 2011 serving approximately 11,000 patients. In this setting, the therapist has latitude for initiative and independent judgment, and provides comprehensive, compassionate, person- and family-centered care to referred Internal Medicine patients, which includes : screening, psychosocial assessment, psychiatric diagnosis and brief, evidence-based treatment. In this role, the mental health therapist also partners with medical providers and medical social work to identify high risk patients with behavioral health and medical comorbidities, contributes to a unified person-centered treatment plan with the goal of improving the patient's overall health and well-being, and facilitates access to other specialty psychiatric and substance use care as needed. The position will require collaboration with physicians, advanced practice providers, nurses, medical social workers, psychiatric consultants, administrators and other staff. The position also involves providing education and support to other clinicians, including Internal Medicine resident physicians, and staff on the Medicine team. This position will be based at the internal medicine residency practice of Strong Internal Medicine at Strong Memorial Hospital. This clinical service is an extension of an Office of Mental Health-regulated clinic which is part of Strong Behavioral Health and the Division of Collaborative Care in the Department of Psychiatry. The mental health therapist provides comprehensive mental health and / or dual diagnosis services for individuals 18 years and older who have a diagnosable mental illness. The therapist functions as the primary behavioral health clinician and is responsible for assessment, treatment planning, therapy / counseling, and education services for patients and families, as well as systematically and continually assesses the patient's mental condition in collaboration with the patient, family, and others in the treatment team. We are seeking candidates with a master's degree in a mental health field (social work, marriage and family therapy, mental health counseling, creative arts therapy) from an accredited training program with major course work in mental health assessment and intervention. Requires NYS licensure as a mental health practitioner (e.g., Marriage and Family Therapist, Mental Health Counselor, Social Worker, Creative Arts Therapist) or limited permit eligible. If licensure has not been obtained upon employment, the individual has 36 months to obtain the NYS license. Responsibilities : - Under general direction and with considerable latitude for independent judgment performs complex psychosocial assessments and diagnostic formulations for patients and their families when indicated. Ensures the development and implementation of comprehensive and individualized treatment plans for patients with clearly defined therapeutic goals. - Conducts the course of individual, family, couple and systems-oriented and evidence-based psychotherapies within the accepted standards of practice as delineated by OMH and the Department, receiving clinical and administrative supervision by senior staff and / or faculty. - Assesses successful completion of therapeutic goals and plans termination of therapy accordingly. - Meets minimum target for clinical productivity set by service director and Division. - Documents treatment in patient's electronic medical record in compliance with Program, Department and Hospital regulations. - Attends clinic staff and team meetings Attends new staff orientation as required by the Division, Department, URMC and discipline. Participates in all required Departmental and programmatic diversity trainings and professional development. - Maintains a thorough knowledge of, and follows, all Discipline, Collaborative Care Division, and Department of Psychiatry Mental Health and Wellness Services policies and procedures, as well as those delineated by OMH. Maintains professional standards and continuing education by reading, attending seminars, workshops and conferences, and staying up to date for the NYS requirements for maintenance of licensure as appropriate. Engages in annual and periodic performance review process with Program Director, supervisor, and clinical chief, where relevant, for the purposes of recognition of strengths and contributions, feedback, and continuous quality improvement relative to performance in the role as well as the Service needs.
