What are the responsibilities and job description for the Case Manager position at The Wellhouse?
POSITION SUMMARY: The Case Manger’s primary responsibilities include overseeing all aspects of The WellHouse’s Mercy House Case Management, supporting each residents’ personal goals to succeed.
ESSENTIAL FUNCTIONS
- Mandatory On-call rotation 1 week a month.
- Assist the Program Director and the Volunteer Coordinator with planning in accordance with the mission of The WellHouse and coordinates the delivery of services among different program activities to increase success, effectiveness, and efficiency.
- Establishes the case management relationship with residents when they enter the home.
- Weekly Case Management is required.
- Encourage resident input, guide the development and implementation of short and long-term goals.
- Refer resident clients to appropriate services to include medical, social, educational, and vocational.
- Regularly works with resident to overcome obstacles that hinder progress.
- Meets weekly with other staff to include Therapists and Program Director to strategize on the issues related to residents.
- Ensure that case management and program activities operate within the policies and procedures of The WellHouse.
- Ensure that program activities comply with all relevant legislation and professional standards.
- Maintain data entry of all case management activities in a timely manner.
- Communicate with residents and others to gain support for the program and solicit input to improve the program.
- Work with other administrative staff to ensure the effective and efficient program delivery.
- Collaborate with the Home Coordinators for the successful and smooth operation of resident activities, chores, and off-site appointments/activities.
- Ensure calendar activities and appointments are scheduled and on appropriate calendars.
- Guide Home Coordinators in providing direction to each resident; assists in meetings with residents relative to issues within the homes.
- Ensures that the Volunteer Coordinator is notified in a timely manner to be able to coordinate transportation, house sitters, and other volunteer provided needs.
- Ensures that the Volunteer Coordinator is notified when a resident is ready for a mentor.
- Perform other related duties as requested by the PD, COO, and CEO.
ADDITIONAL DUTIES AND RESPONSIBILITIES
- Facilitate referral of the resident when required
- Facilitate weekly debrief to multidisciplinary team
- Initiate daily check-in with Home Coordinators
- Ongoing follow-up with Social Service staff is required
- Reports concerns to the Program Director
KNOWLEDGE, SKILLS AND ABILITIES:
- Must be licensed eligible in the state of Alabama.
- Strong judgment and critical thinking skills.
- Ability to maintain professional boundaries with rescues, residents, volunteers, stakeholders, donors, guests, and other WellHouse staff.
- Must possess effective reading comprehension skills.
- Must possess strong customer service skills (courteous, friendly, etc.). Requires the ability to identify and solve problems in a calm, logical manner. Problem solving will include cooperative efforts with the rescues, residents, volunteers, stakeholders, donors, guests, and other WellHouse staff.
- Demonstrated organizational skills, with the ability to manage crisis, deescalate individuals, foster teamwork; multi-task; adapt quickly to change; and the ability to integrate the organization’s mission with required tasks, residents and rescues.
- Excellent interpersonal and communication skills, including the ability to write and speak clearly and effectively, and to listen attentively.

- Demonstrated ability to comply with policies and procedures; uses proven sound judgement and discretion with sensitive information.

- Must maintain eligibility requirements for The WellHouse vehicle insurance coverage. Requires a current and valid unencumbered driver’s license.

EDUCATION AND EXPERIENCE
1. Bachelor’s Degree Preferred: Successful completion of a master’s degree in a social services field preferred.
2. Prefer applicants with at least 2 years of experience in trauma informed services.
