What are the responsibilities and job description for the HOSPICE INPATIENT MANAGER position at The West Texas Rehabilitation Center?
Responsible for overall management aspects of hospice inpatient services.
- Identify need for changes in programs and services for patients requiring inpatient.
- Identify need for changes in programs and services for patients requiring inpatient.
- Develop proposals for consideration by the Executive Director.
- Develop policies and procedures to fill program objectives.
- Oversee scheduling, supervision and ongoing education of CNAs and nurses.
- Participate in marketing and community health activities.
- Participate in program development.
- Participate in on-call schedule as needed.
- Provide guidance in communication, coordination and effective operations relating to patient care.
- Ensures patients requiring the GIP level are transitioned to a lower level of care when symptoms are managed
- Maintain 24 hour, seven days a week availability of RN care for patients/families.
- Maintains line of communication between IDT members and patients/families, physicians, other health care providers and community agencies.
- Assures nursing documentation accurately reflects both care delivered and patient condition and participates in development of plan of care for patients/families.
- Monitors and evaluates implementation of plan of care. Collaborates with other health care professionals when required.
- Assures patient assessments are accurate, complete and timely.
- Conduct interviews of prospective employees and perform annual evaluations.
- Monitor agency compliance with state health department/federal government standards and regulations
- Oversee maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
- Provide input to related admin/computerized systems and monitor related to patient care.
- Reviews complaints and grievances and makes oral/written reports to Director.
- Acts as a resource and provides education for patients/families and IDT members.
- Participate in orientation of new employees.
- Performs other responsibilities as assigned.
This position requires endurance to perform tasks over long periods of work hours and the ability to perform essential functions without posing a "direct threat" in the work place. Must be able to lift 55 pounds and able to assist in a 2-person transfer.
