The Resident Care Manager, in collaboration with the Residence Administrator, is responsible for and manages all clinical aspects of resident care which includes, but is not limited to, supervising the professional care staff and the Resident Assistants and developing and maintaining department performance standards. Provides risk management direction and support to assure compliance with regulatory agencies. Develops and coordinates the specific resident care plans to meet health needs and to promote wellness. The Resident Care Manager is also designated as the “acting administrator” when the Administrator is absent from the community at the same time.

Bottom Line: The Resident Care Manager is responsible for doing anything that assures the clinical needs of our residents are met through the highest standards of delivery and in keeping with “invisible healthcare with dignity”.

Smaller Details Interpret, implement and maintain resident care policies and procedures. Effectively communicate this information to all resident care staff to assure compliance.

  • Conduct an assessment of each new resident on date of admission.
  • Conduct an assessment on each resident every 90 days. Notify family/POA of change in level of care. Collaborate with the Administrator in the final determination on a resident’s eligibility for continued residency.
  • Submit a report to Administrator on changes in levels of care or other resident-related issues by the 10th of each month.
  • Complete all resident assessments as noted in the regulations.
  • Coordinate a specific care plan for each resident within 48 hours of admission.
  • Communicate with the physician regarding issues that keep a resident “at risk”. Notify the Administrator of “at risk” issues as they occur.
  • Inform any family members and physician of any changes in resident’s condition as they occur.
  • Monitor on a daily basis the medication system for the community and for each individual resident. Assure the safety and integrity of the system with respect to guidelines and regulations.
  • Monitor nurse compliance on shift narcotic count daily.
  • Monitor resident health and dietary needs and supervise care provided to residents. Coordinate the delivery of all care and services with the physicians, dietitian, Administrator and the Dining Services Manager.
  • Chart a monthly summary on each resident and chart nursing activity on each resident as needed. Utilize accepted charting guidelines to ensure accuracy and compliance. Train and monitor all designated staff relative to charting guidelines.
  • Complete all incident reports on a timely basis and forward a copy to the Administrator daily and to the appropriate state regulatory agency within the specified timeline. Fax copies of all incident reports to the CSM Director of Operations on the 1st of each month.
  • Conduct floor duties a specific number of hours per week as established by the Administrator.
  • Conduct monthly chart audits to assure the community is in compliance with regulations.
  • Complete resident care waivers as necessary.
  • Advise the Administrator when a resident needs an alternative care setting.
  • Review daily all emergency calls by residents for assistance and complete a review of the treatment and handling of each emergency by staff and submit to the Administrator within the same day. Conduct additional staff training if warranted by the handling of an emergency.
  • Supervise and manage all resident care personnel. On a timely basis, inform the Administrator of personnel issues.
  • Develop and maintain work schedules for the resident care staff members to assure the appropriate service delivery.
  • Review staffing levels, patterns and manpower requirements on a regular basis. Confer with the Administrator to assure the staffing program supports resident needs, levels of care and budgetary guidelines for resident services.
  • Instruct resident care staff on the “invisible healthcare” guidelines and assure that all resident health needs are being met within these guidelines.
  • Assure, through management oversight, that resident care staff members follow all policies, procedures, and health and safety standards.
  • Assess residents' functional status and social, psychological, and spiritual needs on an ongoing basis, but especially prior to move-in and after hospital stays.
  • Assure that care plans are completed and arrange services and support to meet resident needs. Refer residents to community-based services and medical services as necessary.
  • Implement approaches and services to maintain or enhance resident independence.
  • Develop and maintain documentation in resident files pertaining to problems or progress issues presented by residents, families and employees. Documentation must be complete, thorough, and objective.
  • Coordinate the orientation of all resident care staff members and document the orientation process. Support other departments in orienting and training employees on resident care-related items (i.e., resident rights, safety, etc.). Ensure that all training is in compliance with regulatory agency requirements.
  • Conduct monthly in-services for all resident care staff members. Also, conduct additional in-services to support changing resident health needs.
  • Complete annual performance evaluations and salary reviews for all resident care staff members. Recommend pay changes to the Administrator.
  • Train or coordinate the training of staff on health and safety standards.
  • Interview, screen and select new resident care personnel.
  • Recommend changes in internal methods or procedures to the Administrator.
  • Direct the maintenance of all resident care records related to compliance issues.
  • Maintain current knowledge of policies and procedures as related to position or employment.
  • Complete, obtain required approvals, and submit, in a timely fashion, all paperwork as related to the position.
  • Follow established safety regulations and use equipment and supplies in a safe manner.
  • Support and promote the Community vision and mission statement.
  • Maintain confidentiality of all resident or employee information as required.
  • Act as liaison between other departments and managers.

Additional Duties

  • Make oral/written recommendations to the Administrator regarding the operation of the department or the community.
  • Attend required or assigned training and development sessions.
  • Participate in formal and informal quality improvement efforts which include tracking and trending data.
  • Promote and solicit quality improvement suggestions from staff and advance to Administrator for consideration and feedback.
  • Report all unsafe equipment or hazards to the Administrator.

Qualifications

  • Licensed Practical Nurse (Required)
  • Experience in a position involving management or supervision of direct care staff.
  • Experience providing care to a senior population required.
  • Demonstrated knowledge of regulatory agency standards and guidelines.
  • Ability to work as an effective member of the Community management team, prioritize work and meet objectives and timelines.
  • Strong verbal and written communication skills; good meeting facilitator skills.
  • Ability to work independently in all aspects of position.
  • Community compatibility – must be able to coordinate, motivate, and supervise resident care staff, and work closely with Administrator and other management team members.
  • Maintain current knowledge of industry regulations, professional standards and procedures as related to community population.
  • Ability to meet established deadlines and to provide on-going progress reviews to Administrator.
  • Ability to effectively develop and present staff training and development topics as related to nursing issues.

Physical Requirements

  • Ability to walk and work standing for 8 hours (per work day).
  • Ability to work rapidly for 2-hour blocks of time within the workday.
  • Ability to work under time pressure for 8 hours (per workday).
  • Ability to walk on flat dry and wet surfaces.
  • Ability to use standard keyboard and to work with computer monitor or screen.
  • Ability to operate all office equipment (i.e. fax, computer, calculator, telephone, photocopy machine, and printer).
  • Ability to use the standard telephone.
  • Ability to lift, push or pull up to 30 lbs., using proper body mechanics or available equipment.
  • Ability to bend or kneel to perform assorted tasks related to resident services.
