LOCATION: Santa Fe Springs, CA
SCOPE OF RESPONSIBILITY:
Schedule: 10am-7pm Monday-Friday
The primary purpose of the Shelter Case Manager is to assist program participants break the cycle of homelessness and work towards permanent housing. Shelter Case Manager provides day-to-day support to families residing at our shelter facility and provides linkages to supportive services. Support services include medical, educational, social, prevocational, rehabilitative, employment or other needed community services for eligible participants. Targeted population includes children (Birth to age 18) and their families. Shelter case manager works collaboratively with shelter staff and program staff to bridge any gaps in services.

JOB DUTIES:
- Conducts intakes and exits with families entering shelter.
- Case manager will practice Trauma Informed, Housing First, Harm Reduction, and Low Barrier
- Completes and submits progress notes in a 72-hour timeframe utilizing the agency’s electronic record system (HMIS).
- Provides “light touch” case management services such as assist families in preparing for permanent housing transition.
- Collaborating with clients to develop housing plans to address short-term and long-term housing goals.
- Supports with enrolling and exiting participants from Interim Housing Program in HMIS
- Supports in serving meals for shelter families with the support of Shelter Manager
- Supports with creating corrective action plans with families.
- Supports with monthly fumigation.
- Assist with safety walk throughs of the facility.
- Assists with identifying internal and external resources to assist with multiple stressors as it relates to homelessness, domestic violence, substance abuse issues, etc.
- Adopts a “whatever-it-takes” approach in assisting client to retain housing and independent living.
- Provides quality assurance and complete quarterly client file review.
- Aids shelter staff to address crisis situations and ensures the Grievance Process adheres to safety and housing practice protocols.
- Maintain confidentiality in matters of agency operation, personnel, and clients.
- Exercise good judgment in performance of all duties and responsibilities, including maintaining good boundaries and professional conduct.
- Other duties as assigned by Shallow Subsidy Supervisor.
- Bachelor’s degree in social work, psychology, or other mental health related discipline.
- Minimum of 2-4 years of work experience related to providing Housing Navigation or rehabilitative services in a mental health setting.
- Monday-Friday Day Schedule (10am-7pm)
