Position Summary: The primary function of this position is to facilitate admissions and discharges in the skilled nursing facility. Complying with all federal, state and local standards, guidelines and regulations which govern the facility, and, as directed by the Director of nursing Services and/or the Administrator so as to ensure each resident’s medical, psychosocial and spiritual needs and wishes are met.
Duties & Responsibilities: Admittance and Discharge Care Coordinator
General: 75%
- Coordinate all nursing aspects of admission, including but not limited to the following: admission physician orders, MAR’s, TAR’s, head to toe assessment, risk assessments, follow-up appointments and labs, complete green resident care sheet, ensure floor nurse and nursing assistants are aware of care needs.
- Verify admission orders with primary care physician
- Development of resident initial interdisciplinary plans of care.
- Follow-up and implementation of interventions for all areas of concern identified during admission and completion of risk assessments
- Coordinate all aspects of discharges; including but not limited to the following: complete discharge medication list, ensuring interdisciplinary discharge has been completed by all departments, notify primary physician of discharge, schedule follow-up appointments, obtain scripts for medical equipment needs, send all relevant information to primary care physician.
- Attend Weekly Medicare meeting.
- Meet with therapy, social workers, and RNAC’s to determine upcoming discharges and resident needs to ensure safe discharge
- Completes various documentation and reports as directed.
- Assist nursing supervisor with daily tasks
- Maintains a safe work environment and exhibits safe practices, assuring resident safety.
Administrative: 25%
- Attends all nursing department meetings, educational programs, etc
- Actively participates in overall facility management and makes recommendations for quality improvement.
- Reports any supply/equipment needs, repairs, etc., to the Director of Nursing Services.
- Reviews nursing policies and procedures, makes recommendations as warranted.
- Adheres, upholds and enforces all regulations and recommendations of the Department of Health, the Health Care Financing Administration, the Department of Public Welfare, etc
- Participates on various committees as assigned.
Miscellaneous/Other (Exhibited 100%)
- Assumes all other duties as assigned.
- Maintains a professional, customer-oriented demeanor at all times.
- Works within a team concept and is thus cooperative with co-workers and peers.
- Maintains confidentiality of information.
- Represents the facility in a positive manner.
Qualifications
Educational and Experience Requirements:
- Graduate from an accredited school of nursing and in possession of a valid license issued by the Commonwealth of PA.
- Minimum of one (1) year experience in a long term care setting.
- Current certification in cardiopulmonary resuscitation.
- Ability to read, write and speak English.
- Ability to follow oral and written direction.
