Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring compliance with regulatory requirements while providing exceptional patient care. This position requires a strong background in nursing, medical office operations, and familiarity with both inpatient and outpatient settings. The MDS Coordinator will be responsible for coordinating the assessment process for residents, ensuring accurate documentation, and collaborating with interdisciplinary teams to optimize patient outcomes.
Responsibilities
- Conduct comprehensive assessments of patients to determine their needs and eligibility for services.
- Collaborate with nursing staff to gather necessary medical records and documentation.
- Ensure accurate completion of Minimum Data Set (MDS) assessments in accordance with federal and state regulations.
- Participate in discharge planning processes to facilitate smooth transitions for patients.
- Maintain up-to-date knowledge of ICD-10 coding and its application in patient care documentation.
- Work closely with healthcare providers in both inpatient and outpatient settings to promote continuity of care.
- Review and analyze patient data to identify trends and areas for improvement in care delivery.
- Provide training and support to staff regarding MDS processes and documentation standards.
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with relevant experience in a medical office, hospital, or clinic setting.
- Strong understanding of discharge planning processes and patient care coordination.
- Familiarity with medical records management and regulatory compliance standards.
- Proficiency in ICD-10 coding is preferred.
- Excellent communication skills, both verbal and written, with the ability to work collaboratively within a multidisciplinary team.
- Strong organizational skills with attention to detail and the ability to manage multiple tasks effectively.
- Previous experience as an MDS Coordinator or similar role is a plus but not required.
