What are the responsibilities and job description for the Patient Program Coordinator position at The Women's Breast & Heart Initiative?
The Women’s Breast & Heart Initiative, FL Affiliate Patient Program Coordinator is responsible for helping to successfully execute our lifesaving and transformational mission through functions in three areas: program administration, data management, patient navigation and care coordination.
Program Administration:
· Maintain, update and reconcile weekly duty schedule
· Maintain and update participant/patient files
· Prepare educational outreach materials
· Assist with preparations for annual events
· Assist with educational package assembly
· Assist with outreach set up and implementation
· Prepare and manage patient/participant intake sheets
· Conduct Health Educational Rallies
· Manage outreach material inventory
· Order promotional items for outreach
Office Administration:
· Answer incoming calls, routing to the appropriate destination
· Send and receive all e-mail from the Women’s Breast & Heart Initiative e-mail account
· Order and maintain office supplies
· Maintain and update position procedure manual
· Submit and reconcile weekly schedule
· Submit and reconcile weekly schedule
Data Management:
·Update and maintain patient/participant database
· Update and maintain office and outreach expense database reports
·Manage outreach results reports and screenings completed
Patient Navigation:
·Support Workplace Wellness Educational Rallies
·Coordinate Workplace Wellness screening and wellness day appointments
·Coordinate follow up screening appointments in accordance with WBHI Policy
· Manage and track patient/participant outcomes
· Maintain patient/participant call log
· Manage and update past patient/participant screenings and reminders (temporary)
· Perform related duties and responsibilities as required
Education:
·Minimum Bachelor’s Degree in administration or related field
Experience:
·Minimum of two years successful work experience of management with a not-for-profit organization
· Speak fluent English and Spanish
·Demonstrated oral and written communication skills
·Ability to work with patients, volunteers, committees and board members in the implementation of strategies, plans and projects
· Successful track record of managing multiple projects with a diverse customer base
· Must have excellent organization, communication, interpersonal and telephone skills
· Demonstrated oral and written communication skills
· Excellent Time management
· Ability and willingness to travel
· Computer literate
· Computer and technical knowledge; proficient in Microsoft Office applications and Salesforce
· Ability to prepare clear and concise correspondence and reports
· Ability to read, analyze and interpret data and reports
· Ability to respond effectively to the most sensitive inquires or complaints
· Ability to calculate figures and amounts, including percentages
· Ability to interpret instructions in a variety of forms
· Interact well with others and is able to maintain positive working relationships throughout the community with diverse groups of people and within a wide variety of work environments
·Ability to work with volunteers and committees in the implementation of strategies, plans and projects
· Successful track record of managing multiple projects with a diverse customer base
· Salesforce experience a plus
Transportation:
·Must have and maintain a driver’s license valid in the State of Florida and own a reliable insured automobile for use during work hours.
In addition to our local focus area of South Florida, WBHI is replicating its successful outreach program in other areas of Florida and beyond. Work shifts may vary with some extended hours due to event campaigns or special events.

Pay: From $50,000.00 per year
