Director of Social Services Care Coordinator

We are looking for a Director of Social Services Care Coordinator to:
- Serve as an essential Leader to the interdisciplinary team in developing and maintaining a patient centered plan of care addressing patient/residents identified needs and verify all disciplines are addressing planned interventions to ensure a safe transition to the next level of care.
- The Director of Social Services Care Coordinator will deliver timely NOMNC & ABN Notices as well explaining and expediting the appeals process.
- Complete all appropriate sections of the MDS timely and accurately.
- Maintain Quality Improvement efforts, and regulatory compliance as it relates to NOMNCs, PASRR, Advance Directives, Care Plan meetings, and Ombudsman notification.
- Complete all appropriate sections, required documents of MDS, ensuring it meet the needs of patients throughout care transitions.
- Demonstrate and promote a positive, collaborative, and caring service excellence team environment.
Qualifications:
- Bachelor's degree in social work or human services
. 1 year of experience in working in a healthcare social service setting with direct indiviuals
. Strong customer service, teamwork and communication skills.
