Job Title: Personal Care Provider/ QMAP
Full Time, Part Time or PRN (as needed).
Day and evening shifts available.
Some weekends and some holidays.
Location: 900 Centre Ave. Fort Collins, Colorado

About the Role:
As a Personal Care Provider/QMAP at New Mercer Commons, you will:
Help residents build and maintain meaningful relationships by facilitating group activities.
Allow residents to maintain their dignity by assisting with activities of daily living such as bathing, dressing, meals, and maintaining a clean-living environment.
Accurately document and track care plans including restorative care under the direction of various therapists.
What We're Looking For:
Experience:
Interest in geriatrics required; previous experience preferred, but not required.
Must be willing to attend QMAP classes with the follow-up expectations of administering medication.
Skills:
Ability to read, write, speak, and follow oral instructions in English.
Attention to detail in all aspects of care provision.
Capability to work both independently and in a team environment.
Attributes:
Compassionate and empathetic nature with a desire to care for others.
Reliable with a strong work ethic.
A team player who fosters and promotes a positive and supportive work environment.
Our Total Rewards package includes a salary range of between $17 - $22/hr. depending on experience and includes shift differential pay.
