POSITION SUMMARY

Clinical Pharmacist will be located within The Wright Center for Primary Care and focus on providing longitudinal, pharmaceutical care to patients within the clinical settings. The Clinical Pharmacist will also provide education and outreach to physicians, residents and other providers on prescribing guidelines, alternative drug therapies, and risk mitigation.

ESSENTIAL JOB DUTIES & FUNCTIONS

While living and demonstrating our Core Values, the Clinical Pharmacist will:

  • Consult with the patient’s physicians and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals.
  • Evaluate drug therapy, safety and efficacy through direct patient interaction and review of electronic medical records.
  • Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions in collaboration with other members of the health care team. 
  • Assist with education, documentation and special projects surrounding safe opioid prescribing. 
  • Provide medication counseling to patients and caregivers to include generic counseling.
  • Provide educational in-services on medication-related topics to healthcare providers on provider transition of care, as well as trainees and students on topics such as medication alternatives based on patient affordability.
  • Develop and participate in pharmacy-related quality improvement and medication management initiatives.
  • Work with infectious disease team and provide oversight and leadership for the 340B program to ensure to optimize its importance.
  • Coordinate and manage medication and sample inventory for clinics.
  • Work with Primary Care team in meeting patient’s transitional care goals (may include home visits, practice encounters, etc.).
  • Assist with claims/prior authorization issues.
  • Assist patients in enrollment of patient assistance programs.
  • Research and respond to drug information questions.
  • Protect patients and technicians by adhering to infection-control protocols.
  • Maintain pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
  • Provide education to residents such on quality initiatives around cost conscious care and education to learners regarding medication alternatives around socio-economic determinants of health (SEDH).
  • This position will work thirty (30) hours per week. Hours will be based on ten (10) hour days schedule will be Tues/Wed/Thurs one week and alternating Wed/Thurs/Sat the following week. This position will be required to work 1-2 holidays per year. This position will be based out of the WCPC Mid Valley, until the new clinic in Scranton is constructed.
  • Perform other duties as assigned.

QUALIFICATIONS REQUIRED

  • Bachelor of Science in Pharmacy or PharmD
  • Completion of a Pharmacy Residency or 2 years of clinical experience
  • Proficiency in all Pharmacy computer systems, automated dispensing software and systems
  • Excellent interpersonal, verbal and written communication skills


REQUIRED LICENSES/CERTIFICATIONS

  • Current licensure by the PA State Board of Pharmacy
  • CPR Certification 
  • Valid PA Driver’s License
