Job Type Full-timeDescriptionPOSITION SUMMARYThe Center of Excellence (COE) Case Manager is responsible for providing comprehensive case management services to individuals with a history of addiction and co-occurring disorders, particularly individuals with opioid use disorders (OUD), and providing specialized support to The Wright Center clinical staff. Responsibilities include a range of client-centered services that link clients with medically assisted therapy (MAT), primary care, psychosocial, and other services. Behavioral health case management ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the clients' and other key family members' needs and personal support systems. This position includes client-specific advocacy and / or review of utilization of services, including warm handoffs from inpatient, outpatient, and emergency. This position requires flexible working hours including evenings, weekends and holidays.REPORTING RELATIONSHIPSThis position reports to the Lead COE Case Manager. No other roles report to this position.ESSENTIAL JOB DUTIES AND FUNCTIONSWhile living and demonstrating our Core Values, the COE Case Manager will : Demonstrate professional interdisciplinary skills and provide comprehensive Behavioral Health Case Management services to clients (including treatment adherence counseling)Develop and implement comprehensive, individualized service plansFollow up with assigned clients to provide support, monitor progress on service plan, and provide access to resourcesProvide ongoing support and resources to clients in unstable living situationsWork with interdisciplinary team and clients to develop individualized case management plansMaintain client confidentiality per The Wright Center policy and HIPAA standardsServe as client advocate; Defend and preserve the rights of patients pertaining to all aspects of their care, knowledge, and privacyComplete client home-visits when necessary to accomplish service plan goals and maintenanceActively participate in client and program-related trainingsProvide for continuity of effective client and family care by acting as a liaison / communication link between the interdisciplinary team and staff of substance abuse agencies, correctional institutions, specialty offices, visiting nurse associations, and local service organizationsWork with the interdisciplinary team to identify unmet needs that may impact adherence (homelessness, substance abuse, psychiatric illness, acute medical problems, lack of or inadequate health care benefits, etc.) and assist team in prioritizing those needsParticipate in interdisciplinary team meetings as scheduledAssist program staff in data collection and fulfilling grant requirements; Perform data entry as required by grantsMaintain an information / referral list of community servicesDemonstrate leadership and professionalism to other staffPerform other duties as assigned RequirementsREQUIRED QUALIFICATIONSMeet The Wright Center for Community Health and its affiliated entity The Wright Center for Graduate Medical Education EOS© People Analyzer ToolBuy in and experience working in the EOS® model (strongly preferred)Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational prideHigh school diploma or equivalent required. Bachelor's degree in social work, psychology, sociology, counseling, or other related field preferred. LSW a plus.3-5 years' experience in substance abuse care and serviceCurrent BLS (Basic Life Support) requiredUnderstanding of medical and psychosocial issues related to opioid use disordersHave knowledge of, and contact with, health care entities, social service agencies and public entitlement programs in immediate and surrounding communitiesHave knowledge of service costs and budgetary parameters and be fiscally responsible in carrying out all case management functions and activitiesAbility to work with diverse populations and with critical and emergency situationsSensitivity to cultural and economic differences
