POSITION SUMMARY:
As a Community Health Worker (CHW), you will be an integral member of the Social Responsibility team. CHWs will perform screening,referrals, counseling, and care management, including connections to appropriate health insurance and housing resources. Using the SDOH screening tool, CHWs will identify the needs of individuals and provide a warm hand-off to appropriate health and social service resources. CHWs will contact resource organizations using established resource guides and existing hospital and community services; resources may include transportation, food insecurity, housing, economic stability, etc. All referrals will be documented. CHWs will implement evidence-based chronic disease prevention programming, such as the Diabetes Prevention Program, LiveStrong for people recovering from cancer, and Rock Steady Boxing for people with Parkinson’s. Each CHW will also manage a caseload of Druid Hill community members Hill members who are participating in evidenced-based -partner health and wellness interventions that are being implemented at Druid Hill Y. CHWs will not only lead quarterly Y Community Health Events but will attend a wide variety of West Baltimore community events to build Y CHW's presence. CHWs coordinate with the Y Fresh Mart coordinator to address community food insecurity. They will attend Y on the Fly events to ensure that physical fitness and healthy food information are disseminated to the community.
As a Community Health Worker , you’ll contribute to the Y and to the broader community we serve by creating experiences that help people feel a greater sense of health and well-being, purpose, acceptance, and belonging. This includes supporting volunteerism.

ESSENTIAL FUNCTIONS:
∙ Assists clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing. Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
∙ Documents all client encounters and contracts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
∙ Educates client on the proper use of the Emergency Room, and provides information for alternatives. Coaches patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
∙ Assists clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
∙ Support Food support the implementation of the Y’s Food access program
∙ Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
∙ Facilitates communication and coordinate services between providers and the clients/patients. Coordinates and monitors services, including comprehensive tracking of clients' compliance in relation to care plan objectives.
∙ Writes grants and other proposals to secure extramural funding for the CHTM Nanofabrication Facility.
∙ Travels extensively to client homes, community locations, various agencies, and other outreach destinations.
∙ Performs miscellaneous job-related duties as assigned.
