    Summary : POSITION SUMMARY :

    The Director, Hospice and Palliative Care is a key member of the continuum of care leadership team and responsible for setting strategies and the future direction of ThedaCare' s Hospice and Palliative Care services in partnership with the physician medical director. Is responsible for all hospice and palliative care delivered including financial management and oversight, clinical quality, engagement, research, work force management, and professional practice. Provides leadership and creates a culture that supports the delivery of patient-centered, specialized, and evidence-based care across the continuum through an inter-professional approach to treatment, research, education, and advocacy. Contributes to the goals of the organization and patient care by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Wisconsin Boards of Nursing, Social Work , and all other associations related to members of the interdisciplinary team and ThedaCare policy. Is responsible for patient outcomes that meet the high quality of care, promotion of a culture of innovation, and a commitment to professional development.

    Job Description :

    KEY ACCOUNTABILITIES :

  • Provides strategic planning, program development, labor management, and overall operational administration of the departments.
  • Effectively directs team members in a manner consistent with ThedaCare policies and values. Ensures the recruitment and retention of competent team members to meet the needs of the business operations of the departments.
  • Oversees and ensures accuracy of time and attendance and payroll practices.
  • Collaborates with the chief nursing officer, participates in the development, implementation, and monitoring of the nursing strategic plan and other business plans that impact professional nursing practice. Participates with the physician medical director in establishing vision and strategic direction. Supports and establishes goals that connect the vision back to the hospital and primary care and effectively translates strategy into a clear vision.
  • Develops an environment that is supportive of shared decision making, shared governance, and an engaged workforce, and demonstrates personal and professional accountability in the development of high functioning teams.
  • Supports research and innovation to promote palliative and hospice care. Uses evidence-based data to influence safety innovations and improve quality outcomes.
  • Maintains a communication system that interfaces with the medical staff and other leaders and departments to ensure quality patient care. Impacts change through credible influence and negotiation with other senior leaders.
  • Anticipates challenges or barriers, proactively seeks solutions, and acts promptly and decisively to transform ideas into improved outcomes and sustained results. Effectively leads and manages change at ThedaCare, measures the impact of changes to help quantify the effect of improvement, and develops new tools to assist in performance monitoring.
  • Leads and supports ongoing activities that promote safe practices, safe environments, and best outcomes. Develops quality improvement processes that include goals (clinical care and financial performance) and defined performance measurements, and monitors division performance against the appropriate national benchmark.
  • Promotes patient-centered care and reduces unnecessary medical financial costs to improve the medical cost burden on any individual, family and community.
  • Provides management and oversight for financial and budgetary performance including explanation and justification of actual vs. budget variations of core cost centers. Directs, coordinates, and or participates in the preparation of the annual operating and capital budget for the areas of responsibility assigned.
  • Ensures compliance with regulatory agencies, develops policy, procedures, and protocols that support the hospice and palliative care scope of services in partnership with the physician medical director, and provides advice and counsel
  • Develops as needed and establishes a Hospice and Palliative Care program that spans across all care delivery sites (Acute Care, Ambulatory Care, and Home Care). This program will identify eligible people at the optimal clinical time to provide the care when and where needed. Improving quality of life and reduce inappropriate medical utilization and distress during their Hospice and Palliative Care experience.
  • QUALIFICATIONS :

  • Bachelor of Science degree in nursing
  • Eight years of progressive responsibility in healthcare leadership
  • Five years of leadership experience to include leading teams and / or projects
  • Current Wisconsin RN license
  • PHYSICAL DEMANDS :

  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
  • Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
  • WORK ENVIRONMENT :

  • Ability to move throughout hospital departments and floors on a daily basis
  • Frequent exposure to sharp objects and instruments
  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents
  • Standing and / or walking for extended periods of time
  • Transporting, transferring, positioning patients and / or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed
  • Occasional contact with aggressive and combative patients
