Summary :
The Supervisor of Psychosocial Services provides quality psychosocial care to patients and coordinates care with the medical team while consistently applying the social work code of ethics. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice defined by the Wisconsin Board of Social Work and ThedaCare policy. Is responsible for patient outcomes that meet high quality.Job Description:
KEY ACCOUNTABILITIES:
Provides direct supervision for hospice social workers, hospice and hospital chaplains, hospice bereavement services and hospice volunteer service staff.
Identifies patients with complex care needs, conducts patient psychosocial status assessment, establishes a rapport with patient/family, and actively participates in creating and implementing a plan of care to achieve outcomes
Monitors compliance with standards of care by observation, verbal reports, audits, and written documentation, with feedback loops to department leaders, providers, and team members.
Demonstrates emotional intelligence and confidence and maintains composure during conflict. Serves with integrity and sets a positive example at all times.
Provides input and supports the department leaders in interviewing and hiring, appraisal performance, mentoring, motivating, coaching, and retention of team members.
Acts strategically, understands and can manage change, and assists team members through transitions. Advocates for stakeholders to meet strategic goals, and assists team members in understanding how their day–to-day jobs contribute to organizational strategy.
Provides feedback to peers in a constructive manner, keeps information confidential, and develops and maintains lateral, cross-boundary relationships. Utilizes effective oral and written communication skills to collaborate with the health care team as needed to enhance quality and care.
Plans for the total care needs of the patient in collaboration with the interdisciplinary team to provide the highest quality of care and clinical outcomes
Demonstrates clinical expertise in the clinical specialty of assigned responsibility with the ability to perform all functions of the professional medical social worker that are age appropriate and culturally specific.
Provides consultation and maintains positive relationships with all inter-professional team members, collaborating to problem solve and improve patient care.
Collaborates on team schedules, monitors staffing levels, and provides input to the budget preparation/monitoring and the identification of workload variability to predict and plan for appropriate staffing to enhance productivity/efficiency and maintain quality care. Delegates team member duties in relation to patient needs, and manage evolving demands from team members, physicians, equipment, and scheduling.
Serves as a consultant, patient advocate, and liaison for patients and families in areas of social work needs and complex psychosocial issues that could include facilitation of specialty-specific psychoeducational support groups in some care settings.
Supports efforts to decrease hospital readmissions when possible and returns patient to the community with appropriate resources.
Applies critical thinking to assess, plan, and implement day-to-day operations, and communicates updates to team members
Oversees and ensures the provision of education and training as appropriate, and participates with leaders in quality improvement activities.
QUALIFICATIONS:
Master’s degree in social work
Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW)
American Heart Association Healthcare Provider Basic Life Support (BLS)
Two years of experience leading projects, a council/committee, and/or clinical improvement work or progressive leadership responsibilities.
PHYSICAL DEMANDS:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT:
Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock
