About the Job
We are seeking an experienced and skilled Medical Director to join our team. As a Medical Director at Theoria Medical, you will be responsible for leading our team of healthcare professionals and ensuring the highest level of care for our patients.
Key Responsibilities:
- Lead the medical directorship function, overseeing skilled nursing facilities and participating in administrative and quality initiatives
- Collaborate with nurse practitioners and physician assistants to manage day-to-day resident care
- Provide high-quality, patient-centered primary care services by rounding on patients residing in SNFs
- Diagnose and treat patients with acute illness, chronic diseases, and conditions
- Document patient information in our proprietary electronic health record system, ChartEasy
- Participate in our Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives and programs
