Job Description:
We are seeking a highly skilled and experienced Medical Director to join our team at Theoria Medical. As a key member of our post-acute care team, you will be responsible for providing medical leadership and direction to our skilled nursing facilities (SNFs).
Responsibilities:
- Serve as the attending physician for residents residing at each facility.
- Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care at each facility.
- Rounding on patients residing in SNFs to provide high-quality, patient-centered, comprehensive primary care services.
- Participate in quality improvement initiatives and programs, including Chronic Care Management, Transitional Care, RPM, and others.
Requirements:
- Experience in post-acute care or similar setting.
- Strong leadership and communication skills.
- Able to utilize electronic/online medical tools such as EHR systems.
