MUST HAVE ARIZONA LICENSE FOR COTA / OTR OR IN THE PROCESS OF OBTAINING ONE AND WILLING TO RELOCATE TO ARIZONA.
Requirements :
- Must be a licensed OTR, OTD or COTA in the state of Arizona.
- Arizona Fingerprint Clearance Card.

All bonuses are paid out contingent to minimum consistent caseload of 16 treatments per week during the initial sign on bonus, and at least 1 year following the bonus pay out date. All other bonuses are contingent on therapist providing at least 16 treatments per week on a consistent basis.

Salary : $2,000 - $4,000
