Key Responsibilities and Requirements
- Assess patient needs and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient care transitions
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal care delivery
- Meet the requirements outlined below:
- Active unrestricted AK License
- Active BLS, ACLS, PALS certifications from the American Heart Association
- At least one year of recent experience as an RN in a Long Term Care setting
- Ability to provide at least one supervisory reference
