Job Summary
This role is focused on providing essential support services to individuals with mental illness in a Fountain House/Clubhouse setting.
Main Responsibilities:
- Personalized Support: Deliver high-quality support to individuals who require assistance to manage their mental health.
- Plan Development: Create and execute customized plans to promote independence in daily living skills, socialization, leisure activities, educational, and vocational skills.
- Record Keeping: Maintain accurate and detailed records of services provided to consumers on a daily basis.
- Transportation Services: Safely transport consumers to community services or activities while adhering to safety guidelines.
