Main Responsibilities:
- Support Individuals: Provide compassionate and dedicated support to individuals with mental illness in a Fountain House/Clubhouse setting.
- Develop Personalized Plans: Create customized plans to promote independence in daily living skills, socialization, leisure activities, educational, and vocational skills.
- Document Services: Maintain accurate records of services provided to consumers on a daily basis.
- Transportation Services: Safely transport consumers to community services or activities while adhering to safety guidelines.
Requirements:
- Qualifications: Minimum age: 24 years old; High School Diploma or GED required.
- Clearances: No substantiated findings on the NC Health Care Abuse Registry.
- Driver's License: Valid driver's license required.
