URGENT POSITION
Salary Range: $30 - $40/hour (dependent on experience)

- Collaborate with a multidisciplinary team to develop and implement individualized care plans.
- Advocate for patient needs and connect them with appropriate resources.
- Maintain accurate documentation and uphold confidentiality standards.
Requirements:
- Master’s degree from an accredited school of social work - Required
- Current or eligible for required state licensure - Required
- Meets state-specific requirements, such as LCSW - Required
- Minimum of 1 year of clinical social work experience, preferably in an ESRD setting - Required
- Current CPR certification - Preferred
