Your Impact:
- Assess patients’ mobility, strength, and functional abilities to create individualized treatment plans.
- Implement therapeutic interventions, including exercises, manual therapy, and mobility training, to improve function and independence.
- Collaborate with an interdisciplinary team to ensure seamless, patient-centered care.
- Educate patients and their families on home exercises and strategies for continued recovery.
- Maintain thorough and accurate documentation in compliance with facility and regulatory standards.
- Monitor patient progress and adjust therapy plans to optimize outcomes.
What We’re Looking For:
- Active California PT license (or eligibility for licensure) – Required
- Minimum of 2 years of Physical Therapy experience – Preferred
- Experience in a Skilled Nursing Facility (SNF) setting – Strongly Preferred
- Strong communication and interpersonal skills to foster a positive patient experience
- Ability to work independently while collaborating with a dedicated healthcare team
