Job Title : Physical Therapy Assistant

Experience : New graduates are welcome; SNF experience preferred

Certifications : None specified
Must-Have : Completed prescreen form, resume, and license verification
Job Description :
- Ensure completion and attachment of the prescreen form upon candidate submission.
- Attach resume and license verification, as candidates without these documents will be declined.
- New graduates are welcome, with a preference for candidates with SNF experience.
- Covid test required 72 hours prior to the start date, with results available 48 hours prior to the start date.
- Conduct thorough credentialing, which includes :
- State license verification
- Three references
- County criminal check for all counties lived and worked in over the past 7 years
- Federal criminal check for 7 years or more
- Multi-state criminal search for 7 years or more
- Nationwide sex offender search for 7 years or more
- HHS / OIG and GSA searches
- Social Security Number trace
- 1 Step PPD or Quantiferon or proof of positive PPD and chest X-ray
- SAM search
- DOJC check (rerun every 4 years with BID form)
- Position urgency is categorized as normal.
- Bilingual skills are not required.
- No holiday coverage required.
