What are the responsibilities and job description for the Discharge Planner position at Thibodaux Regional Medical Center?
Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating physical and psychological factors, financial, social, and community resources with a focus on social issues/discharge planning for patients of TRMC.
Educational and/or Professional Qualifications:
1. Bachelors or Associates degree in a healthcare related field, Social Work, or General Family & Consumer Services degree required.
Previous Experience:
1. 1-2 years experience relative to crisis intervention.
2. Previous experience in resource management.
3. Ability to prioritize responsibilities and use appropriate time management skills.
Licenses/Certifications:
**if bachelors degree is in Social Work, must have license as a Registered Social Worker by the State of Louisiana. If degree is in other field, no license is required.
Other Special Training/Skills:
1. Basic computer literacy/knowledge
