Job Description:
We are seeking an experienced Registered Nurse (RN) - Case Manager to join our team on a contract basis in Syracuse, NY. The ideal candidate will play a crucial role in coordinating patient care, ensuring effective discharge planning, and facilitating communication between healthcare providers, patients, and families.
Responsibilities:
- Assess, plan, and implement individualized care plans for patients.
- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Facilitate communication between patients, families, and healthcare providers.
- Advocate for patient needs while ensuring efficient utilization of healthcare resources.
- Review medical records, insurance coverage, and treatment plans to ensure appropriate care.
- Provide education and support to patients and their families regarding care transitions.
- Ensure compliance with hospital policies, procedures, and regulatory requirements.
Requirements:
- Active RN license in New York (or compact state eligibility).
- Minimum of 2 years of RN Case Management experience in a hospital or healthcare setting.
- Strong knowledge of care coordination, discharge planning, and utilization review.
- Experience with electronic medical records (EMR) systems.
- Excellent communication, critical thinking, and problem-solving skills.
- Ability to work independently and collaboratively in a fast-paced environment.
