Job Summary:
RN Care Manager with our Care Management Team at Medical Center in Tarzana, CA.
This is a full-time position that will work 8-hour Variable shifts.
Must have availability to work 8am-4:30pm or 4:30pm-1am with weekend rotations.

Required Qualifications:
- BSN Degree
- Active California RN License
- 2 years’ experience working as a RN in a hospital / inpatient setting
- 2 years’ experience working as a RN directly in Case Management or Discharge Planning in a hospital setting
- Upon hire: Certification in Case Management (ACM or CCM)
Job Type: Full-time
Pay: $150,000.00 - $173,000.00 per year
