Currently, we are seeking an innovative and collaborative individual to join our team as the Director of Community Based Populations. As the Director of Community Based Populations, you will be responsible for the oversight, leadership and delivery of community-based programs including community health, care linkage, and intervention programs. This individual will be responsible for providing leadership, direction and strategic vision for population health in collaboration with the Chief Medical Officer and Senior Leadership to design, develop and implement programs resulting in measurable improvement in population health outcomes.
General job duties:
- Provide direction and oversight for Community Health Worker, Health and wellness programs and outreach initiatives
- Developing, informing, monitoring, and accomplishing the health centers integrated population community health strategy
- Championing integration and articulating the goals and value of our integrated population health model to leadership, associates, and key stakeholders
- Employing and implementing core concepts of population health improvement such as social determinants of health, health services research, epidemiology, and statistics to guide the health plan’s strategic vision
- Utilizing social needs screening tools, risk modeling, and community needs assessments to leverage relevant internal and external data sources to understand the markets population served and translate data into actionable evidence to inform strategic planning, policy, and partnership
- Building, preserving, and prioritizing collaborative, sustainable relationships with public health, social services, local governmental agencies, and community-based organizations
- Overseeing annual program description, population health analysis and program evaluation documents
- Optimize data collection through the Electronic Health Record
- Oversee outreach activities to engage vulnerable populations in service areas
- Collaborates on submitting grant applications related to population health
- Collaborates on Chronic Care Management program for patients
- Responsible for assessing, training, and motivating employees and job applicants. Optimize productivity within the workplace by promoting professional growth.
- As a people manager, respond to all people-related tasks involving new talent, employee engagement and career development.
- Hire new employees, following all applicable policies, procedures and employment law including EEOC.
- Provide regular feedback to direct reports, documenting the feedback in the personnel file. Complete annual reviews and competencies in a timely and effective manner
- On-boarding new team members to their role and department; providing support and guidance throughout the orientation period.
- Be a positive role model for all health center staff.
- Other duties as assigned.

- Community Health Workers, CHW Coordinator(s), RN Care Coordinator(s), Disease Intervention Specialist(s), PrEP Navigator.

Requirements
Qualifications:
- 3 years of leadership experience
- Licensed Social Worker, Registered Nurse required
- Advanced Degree in one of the following is preferred – Social Work, Nursing, Public Health or Healthcare Administration
- Data analytics
- Comprehensive knowledge of health care systems including state and federal policy
- Ability to create and advance strategic initiatives
