Job Summary: We are seeking an experienced Early Intervention Social Worker/Clinician to join our team at Thom Child And Family Services. In this role, you will work closely with parents/caregivers and other professionals to provide individualized early intervention services to infants and toddlers who have or are at risk for developmental delays. You will be responsible for conducting home visits, assessing the child's developmental status, and developing strategies to support their growth and development.
Responsibilities: Key responsibilities of this role include conducting multidisciplinary team assessments, providing direct child development services, developing Individual Family Service Plans (IFSPs), and completing progress notes and reports. Additionally, you will act as a service coordinator for families, ensuring that they receive timely and effective intervention services.
Requirements: To excel in this role, you will need a Master's Degree in Social Work from an accredited school of social work OR Certified or Licensed Social Worker (LCSW, LICSW, LHMC). Experience working with infants, toddlers, and families is highly desirable. You must have excellent communication and organizational skills, as well as the ability to work collaboratively with other professionals and agencies.
