Job Description: As an Early Intervention Social Worker/Clinician, you will be responsible for providing home and community-based early intervention services to infants and toddlers who have or are at risk for developmental delays. You will collaborate with parents/caregivers to develop Individual Family Service Plans (IFSPs) that address the child's developmental needs and support family engagement in the intervention process.
Responsibilities: The key responsibilities of this role include participating in multidisciplinary team assessments, providing direct child development services, developing IFSPs, and completing progress notes and reports. Additionally, you will act as a service coordinator for families, ensuring that they receive timely and effective intervention services.
Requirements: To succeed in this role, you will need a Master's Degree in Social Work from an accredited school of social work OR Certified or Licensed Social Worker (LCSW, LICSW, LHMC). Experience working with infants, toddlers, and families is highly desirable. You must have excellent communication and organizational skills, as well as the ability to work collaboratively with other professionals and agencies.
