Float- Part Time 20 hours/week M-F 8a-2p or 2p-8p
Job Description:
As a Floating Community Living Counselor, you will play a crucial role in assisting and encouraging individuals to realize both their long and short-term goals. You will support their emotional, physical, and personal well-being while respecting their autonomy and values. Key Responsibilities:
- Support individuals temporarily due to staff absences.
- Meet with supervisor as needed in West St. Paul to ensure seamless service delivery.
- Assist individuals with meal preparation and daily living activities.
- Maintain up-to-date knowledge of each individual's service plan and demonstrate competency in all required areas of service provision.
- Log all day-to-day occurrences to track progress and ensure accountability.
- Accompany individuals to medical appointments and community activities, fostering social engagement.
- Administer and log medications as needed and work with medical equipment when necessary.
- Help individuals maintain their homes by assisting with laundry, budgeting, cleaning, and shopping.
- Perform other duties as assigned, contributing to the holistic well-being of the individuals we serve.
Additional Information:
- Schedule: This position is 20) and requires a flexible schedule. The employee will meet with their supervisor weekly to review the needs of the week. Most hours are daytime business hours but may require the very rare weekend or evening shift.
- Travel: This position may require travel within the Twin Cities and surrounding areas, including Apple Valley, Rosemount, Stillwater, Minneapolis, and Forest Lake. A willingness to travel and maintain an open schedule is essential.
- Varied Experience: While no individual will be on a permanent case load, your experience in the field will be valuable as you provide person-centered support to individuals based on their unique needs and circumstances.
Qualifications:
- Ability to lift and carry up to 20 lbs to assist individuals as needed.
- Willingness to commit to and implement person-centered supports that promote independence and autonomy.
- Must be mature, polite, competent, and possess excellent team-playing, listening, and communication skills.
- High school diploma or equivalent.
- Department of Human Services background clearance.
- Valid driver's license, acceptable driving record, and proof of auto insurance.
- Must be 18 years or older.
- Willingness to drive your personal vehicle to transport individuals.
- Proficiency at a high school level in speaking, comprehending, reading, and writing English to communicate effectively with individuals and staff, implement specialized medical and behavioral protocols, and document required information.
- Experience working with people who have disabilities preferred.
