Job Summary
The Integrated Behavioral Health Patient Navigator serves as a liaison for all patients receiving substance use disorder treatment. Specifically, the patient navigator will work with physicians and staff to ascertain the appropriate services the patient needs; schedule the patient appointments with multiple departments; and follow up with the patient to assure compliance. The Patient Navigator supports delivery of the Jefferson Experience by placing emphasis on the prompt and courteous treatment of all patients and visitors.
Job Description
Essential Functions
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Schedule patients for all visits undergoing substance use disorder treatment
- Administers screenings and intake assessments as needed
- Monitor treatment plan adherence
- Triage patient phone calls made to the MAT clinic cell phone
- Coordinate inpatient addiction care as needed
- Develop, maintain, update community resource list for treatment programs
- Monitor the Integrated Behavioral Health work queue & assist with scheduling new patients
- Greets and interacts with patients / visitors in a professional manner, on phone or in person.
- Adheres to HIPAA privacy and security requirements. Ensures compliance with all applicable federal, state, and local regulatory standards.
- Other duties as requested or assigned.

Must be able to communicate courteously with patients and / or visitors.
Willingness to provide and support a positive Jefferson Experience for all patients and / or visitors.
Leadership skills to include proactivity and problem solving.
Ability to establish and develop positive and collaborative relationships.
Epic experience preferred
Education :
Bachelor's Degree in psychology, health administration or other relate fields from an accredited college / university preferred
Experience :
3-5 years of experience in a medical office required
