22043E The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population to facilitate patients through the continuum of care. Coordinates all aspects of discharge planning and establishes a safe discharge plan for assigned patients. Education: Bachelors of Nursing required AND Experience: Minimum of 3 years case management or discharge planning experience required. Manager - Please indicate minimum level of education and experience for this job. Please indicate whether it is required or preferred.
