Per Diem RN/LPN

Thompson House is a small, non-profit, mission-driven rehabilitation and LTC nursing center serving the community and surrounding towns of Brattleboro, VT. We have a ongoing need on our day , evening and overnight shifts and are looking for a LPN/RN to join our team! This position is currently flexible with a one weekend a month requirement! However we may have additional shifts you can add to your work week to increase your hours if desired! The most qualified applicant would be compassionate, a good team player, have a positive mindset, with attention to infection prevention and compliance. Responsibilities:
- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs
- Collaborate with healthcare team members to develop and implement patient care plans
- Assist with the admission, transfer, and discharge of patients
- Document patient information and maintain accurate medical records
- Educate patients and their families on healthcare procedures and self-care techniques
- Monitor and report any changes in patient condition to the appropriate healthcare professionals
- Ensure compliance with all applicable laws, regulations, and policies

Skills:
- Proficient in medication administration and dosage calculations
- Experience working in a nursing home or long-term care facility
- Strong case management skills to coordinate patient care across multiple providers and settings.

Thompson House offers a variety of perks and benefits to our staff. All staff receive a 30 minute, and 2 20-minute paid breaks during each 8 hour shift. The salary range for this position is $30-45/ hour based on experience and shift worked.
