What are the responsibilities and job description for the Staff Nurse position at Thoroughbred Wellness and Recovery?
We are a compassionate and client-centered substance abuse treatment center dedicated to providing a safe and supportive environment for individuals on their recovery journey.
The Staff Nurse is responsible for providing direct client care, medical oversight, and support in a safe and therapeutic environment. This role involves administering medications, monitoring withdrawal symptoms, collaborating with behavioral health professionals, and ensuring clients receive high-quality, individualized care throughout their recovery journey.
The position follows a consistent 12-hour rotating schedule alternating weekly three & four 12 hour shifts with alternating weekends.
KEY RESPONSIBILITIES
Client Care & Medical Monitoring:
· Assess and monitor clients for withdrawal symptoms, mental health conditions, and medical concerns, ensuring that their physical and behavioral health needs are met.
· Perform routine health assessments, including vital signs, symptom management, and ongoing evaluations, while maintaining accurate documentation.
· Develop and implement individualized nursing care plans in collaboration with the treatment team and ensure initial treatment plans are completed upon admission.
· Ensure compliance with universal precautions, infection control measures, and facility safety protocols when providing direct client care.
· Respond to medical and psychiatric emergencies, providing immediate interventions and notifying the physician of any significant changes in client status.
Medication Administration & Management:
· Administer medications safely, including MAT (Medication-Assisted Treatment) when applicable, while educating clients on medication purpose, side effects, and compliance.
· Ensure all medication administration and controlled substance documentation are accurate, following facility policies and state regulations.
· Maintain proper medication counts, report discrepancies, and ensure orders are legible and correctly transcribed if taken verbally.
· Monitor client responses to medication and adjust care plans as needed in consultation with the treatment team.
Crisis Intervention & Client Support:
· Utilize trauma-informed care and de-escalation techniques to manage crisis situations effectively and safely.
· Provide emotional support and therapeutic rapport while maintaining professional boundaries, respecting personal space, and ensuring a safe treatment environment.
· Collaborate with counselors, therapists, and other medical staff to provide holistic client-centered care.
· Assist in conflict resolution and client complaints, ensuring that all physical, biopsychosocial, developmental, and cultural needs are acknowledged and addressed.
Collaboration & Compliance:
· Work closely with physicians, clinicians, and other healthcare professionals to develop and maintain effective treatment plans.
· Adhere to state, federal, and accreditation standards (HIPAA, Joint Commission, CARF, and OSHA). Ensure nursing care aligns with state Nurse Practice Act regulations, facility policies, and accreditation standards.
· Accurately complete admission and discharge processes, ensuring all medical and behavioral assessments are thoroughly documented.
· Participate in interdisciplinary team meetings, quality improvement initiatives, and ongoing staff training.
QUALIFICATIONS
· Education: Graduation from an accredited school of nursing required.
· Experience: Nursing experience in behavioral health, substance use treatment, or detox settings preferred.
· Skills: Strong communication and crisis intervention skills. Experience with electronic health records (EHR) and documentation preferred.
· Licenses/Certifications: Current RN or LPN license as required by state regulations.
CPR certification, de-escalation and restraint certification (training provided upon hire if needed).
