Under the supervision of the Manager of Case Management, the Social Worker assists in meeting the psychosocial needs of patients and enabling patients to utilize medical care in order to achieve their optimal level of health. Assesses patients for discharge planning and assists with referrals to appropriate local/state/ community resources.
The incumbent will carry-out duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital.

Duties/Responsibilities:
- Interview clients to gather information about their backgrounds, needs, or progress.
- Assess each patient’s unique condition, combining data from medical diagnoses, personal interviews, and professional knowledge to establish an accurate picture of overall mental and emotional health.
- Educating patients on the roles of health care team members; assisting patients and families in communicating with one another and to members of health care team; interpreting information.
- Advocate for clients or patients to resolve crises.
- Develops and ensures delivery of patient care including assessment, planning, implementation, and evaluation of patient outcomes.
- Develop comprehensive plans to help patients adjust to medical diagnoses, including recommending support groups and lifestyle changes, and helping patients transition from hospital care to personal healthcare management.
- Research local resources to determine which ones are right for individual patients, considering eligibility requirements and treatment methodologies.
- Assesses patients’ post discharge needs and facilitates the provision of services necessary to meet identified needs.
- Facilitate referral and linkage process to appropriate services.
- Perform other duties as assigned.
Requirements:
- Required: Bachelor's in Social Work (MSW)
- Required: Current license as Social Worker from New Mexico Board of Social Work Examiners.
- BLS Certification Required
- At least 2 years of healthcare or case management experience preferred.
