MDS COORDINATOR (Registered Nurse)

Salary / Exempt:  0.8 FTE 

 

Salary Range:   $69,056 - $78,041 depending on experience

Position Purpose:

The primary purpose of this job position is to coordinate the development and completion of the Individual receiving services Assessment Instrument (RAI) (also commonly referred to as the MDS) in accordance with the requirements of this state and the policies and goals of this care center. The MDS Coordinator will oversee the completion of an MDS that is comprehensive, accurate, standardized and reproducible.

Organizational Responsibilities:

• Adhere to, support and promote St. Francis Health Services’ Mission and Core Values, philosophy, goals and objectives while conducting company business.
• Maintain confidentiality of company and individual receiving services related information at all times.
• Develop, maintain, and update as required, written policies and procedures that govern the day-to-day functions of the department.
• Serve on, participate in, and attend various committees of the care center as appointed by the Administrator.
• Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Director of Nursing or Administrator as required.
• Maintain and periodically update written policies and procedures that govern the development, use and implementation of the MDS and care plan.
• Participate in the development, maintenance, and implementation of the care center’s quality assurance program for the MDS and care plans.
• Evaluate and implement recommendations from established committees as they may pertain to the MDS and care plan functions of the care center.
• Meet with and solicit advice from department supervisors concerning the MDS/care plan functions of the care center.
• Coordinate the planning, scheduling, and revising the MDS, including the implementation of CAA’s, Triggers and care plans.
• Ensure the IDT members that completed a portion of the MDS signs and dates the MDS attesting to the accuracy of their information.
• Ensure the IDT conduct timely individual receiving services assessments and reviews according to the schedule set down by the RAI.
• Complete the electronic submission of MDS records and MDS Error correction.
• Identify when there is a significant change in condition in an individual receiving services’ status and implement a Significant Change MDS Assessment.
• Encourage the individual receiving services and his/her family to participate in the development and review of the individual receiving services’ plan of care.
• Review progress notes to determine if the care plan is being followed.
• Review and revise care plans and assessments as necessary, but at least quarterly.
• Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing the individual receiving services’ comprehensive care plan.
• Track individual receiving services for Medicare eligibility, enroll and deny as necessary, per Medicare guidelines.
• Provide monthly billing SNF sheets.

Regulatory Responsibilities:

• Keep abreast of current federal and state regulations, as well as professional standards of practice, and make recommendations on changes in policies and procedures to the Administrator.
• Pro-actively manage the implementation of changes in policies and procedures. Facilitates staff training when changes occur.
• Understand and follow the Vulnerable Adult and Abuse Policy and reporting procedure.
• Understand and adhere to Individual receiving services Bill of Rights.
• Conduct and coordinate the development and completion of the individual receiving services assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the individual receiving services assessment, including the implementation of CAAs and Triggers. Make recommendations for care center practice changes when rules, regulations and guidelines change.

Education and Experience:

• Must possess, as a minimum, a Nursing Degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state.

• Must respect the individual receiving services’ personal and property rights.
• Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times, hostile people within the care center.
• Must be willing to work beyond normal working hours when necessary.
• Must not pose a direct threat to the health or safety of other individuals in the workplace.
• Able to operate the following equipment:
o Alarms and other safety interventions/devices.
o Call light systems.
o Walkie talkies and/or pagers, if provided by care center.
• Must be able to request and/or accept direction/education as needed to develop new skills.
• Must possess the ability to reach out to other healthcare providers to obtain information needed to complete the MDS.
