We are looking for a passionate Travel Registered Nurse (RN) to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.
POSITION SUMMARY: The Hospice Travel RN case manager will oversee the care of assigned patients by coordinating and providing nursing assessments and care to patients and families in accordance with the interdisciplinary plan of care. The RN is required to travel to assigned agency of the company on a regular basis to provide patient care in agencies in need of permanent staff.
QUALIFICATIONS:
- Graduate of an accredited associate or bachelor’s degree in nursing
- Current state license as an RN and current state driver’s license
- Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred
- Current CPR certification
- Proficient skill using a computer is required, Previous Electronic Medical Records system highly preferred
- Strong interpersonal skills and ability to provide effective support to the patient and family
- Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- Strong communication and organizational skills and high standard of ethics and compliance
