MDSQ Options Counselor (OC)
Primary Responsibility:
Georgia’s Aging & Disability Resource Connection (ADRC) is designated by the Department of Community Health (DCH) as the local contact agency (LCA) to receive referrals and provide options counseling (OC) for individuals wishing to transition from nursing facilities as required by the Centers for Medicare and Medicaid Services under Section Q of the Minimum Data Set (MDS) 3.0.
Summary of Duties:
· Respond to referrals within ten business days of receipt of the referral
· Work closely with nursing and rehab facilities social workers within TRRC 10 county region
· Establishes eligibility, completes screening and refers participants accordingly
· Provides person centered options counseling regarding options and/or services to clients
· Make referrals to any identified programs/services within ten business days of the determination.
· Refer clients to Money Follows the Person and/or Nursing Home Transition Programs
· Complete timely follow-ups with clients
· Provides minimum of four outreaches monthly to all potential referral sources
· Actively serve on Advisory Council and attend meetings as required.
· Submit monthly MDSQ reports to the Division of Aging Services as required
· Presents information in the community (i.e. nursing homes, senior centers, hospitals and other interested parties)
· Document case notes in Wellsky data system a timely manner to meet standard of promptness requirements
· Maintain tracking sheet of referrals and referral sources for reporting purposes
· Coordinates family conference opportunities for client and their family members
· Must become Options Counseling certified as well as AIRS certified
· Maintain certification status as required
· Any other duties as assigned
· Travel to nursing homes for options counseling 50%
· In-services/Marketing to nursing homes or other community partners 10%
· Follow up with clients and or families 10%
· Office work and documenting files 30%
Qualifications and Requirements:
B.A. degree in Sociology, Psychology or related field. Two years’ experience in human services.
Other Requirements:
Pre-employment drug screen; criminal background check; proof of a valid driver’s license; conformity to Three Rivers Regional Commission standards of conduct for employees; professional business appearance and demeanor; appropriate language skills to communicate with a variety of business organizations and people, means and ability to travel throughout a multi-county area and some overnight travel, good work history. Must be able and willing to work a flexible schedule including evenings if necessary. Must be able to lift at least thirty pounds
Preferred Qualifications:
Ability to use assessment tools and technology; knowledge of older persons; Skills in coordinating with and communicating with people making inquiries and with service providers. Experience with working in nursing facilities.
