Your Role
As a Registered Nurse in our home health team, you will provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing. You will work closely with patients, families, and other healthcare professionals to develop and implement individualized plans of care that address the unique needs of each patient. Your responsibilities will include:
- Conducting comprehensive assessments of patients' nursing needs and developing plans of care in collaboration with physicians and other healthcare professionals.
- Providing direct patient care, including administering medications, wound care, and vital sign monitoring.
- Collaborating with patients' families and caregivers to ensure smooth transitions between healthcare settings.
- Maintaining accurate and complete records of patient care, including progress notes and discharge summaries.
