What are the responsibilities and job description for the Hospice LPN Case manager position at Three Rivers Hospice, Inc.?
Three Rivers Hospice office in Mountain Grove, MO is actively seeking a competent LPN to case manage hospice patients. The LPN Case manager will manage a case load on the average of 10 patients, while working with the Interdisciplinary team and attend IDG meeting every 14 days. The LPN Case manager occasionally takes back up on call when the on-call team needs support. LPN should be familiar with the concepts and needs of patients / families who are facing death and dying issues.
The home location will the Mountain Grove office.
