What are the responsibilities and job description for the WISe Bilingual Care Coordinator position at Three Rivers Therapy?
WISe or Washington State’s Wraparound with Intensive Services (WISe) is based on System of Care (SOC) values and is designed to provide comprehensive behavioral health services and supports to Medicaid eligible individuals who are twenty years of age or younger, (herein referred to as “youth”) with complex behavioral health needs and their families. SOC values are family driven and youth guided, community based and culturally and linguistically appropriate. The goal of WISe is for eligible youth to live and thrive in their homes and communities, as well as to avoid or reduce costly and disruptive out-of-home placements while receiving behavioral health treatment services.
This team member coordinates and facilitates the WISe process for youth and families to include advanced care coordination, facilitation of meetings and ensuring the phases and activities of WISe are completed for each client served by CHSW in the WISe program. Has access to protected healthcare information (PHI) or confidential identifiable information (CII), both paper and electronic, as necessary, to perform related job duties and responsibilities.
Essential Functions
- Prepares for Child and Family Team (CFT) meetings to include: developing meeting agenda with assistance of appropriate CFT member(s), scheduling meeting and inviting all CFT members, preparing visual aids or tools to be used to facilitate meeting process
- Facilitates the CFT meeting, guide team process, and be a central point of communication for team and clients served
- Contacts CFT members who are unable to attend a meeting, and gather their input prior to the scheduled meeting date
- Establishes and sustains and effective team culture and engage all CFT members and identify their needs for meeting agency mandates
- Increases “natural supports” in CFT membership and the you/family integration into their community by getting to know the family history, culture, and resources and by helping the youth and family to identify and engage potential supports
- Ensures all CFT members receive an updated copy of the CSCP, documentation of progress, CFT meeting activities, discussions and task assignments as required by WISe.
- Works with Youth and/or Family Support Specialist to identify family support, peer support or other “system” and community resources that can assist the youth in exercising their voice during the CFT process, if needed.
- Completes WISe required documentation by specified timelines including CANS Assessment, Crisis Planning in conjunction with team members, and CSCPs.
- Informs the youth and family of their rights (including Due Process) and obtaining all necessary consents and releases of information
- Advocates for the values of a diverse population within CHSW and the Community
- Performs other duties as assigned by the supervisor
Knowledge, Skills, And Abilities
- Knowledge of family support principles and practice, early childhood development and family development
- Proficient in MS Office
- Demonstrated successful community organizing or ethnic community outreach experience
- Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community
Working Environment
Work is split between clients’ homes, office, and car. The following list is representative of what employees encounter while performing the essential functions of this job:

- Home visits involve environments that may include pets, residual smoke, and other typical household hazards like dirt and other spills (Note: we do not allow smoking during visits)

Physical Demands
- While performing the essential duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer and telephone keyboard; occasionally to reach, stoop and kneel
- Specific vision abilities required by this job include close vision requirements due to computer work
- Ability to sit on the floor, crouch, or kneel regularly during home visits
- Ability to lift children if necessary
- Ability to sit in car frequently
- Regular, predictable attendance is required
