What are the responsibilities and job description for the LPN position at Three Rivers Treatment Center LLC?
The LPN understands and adheres to the standards of nursing. Provides direct care for residents and students. Serves actively and constructively as a member of the multi-disciplinary treatment team to determine and provide the best possible care. Ensures the safety and wellbeing of each resident / student. Supervises medication schedules and administers medications in accordance with established safety, Facility, and regulatory requirements with minimum medication errors. Ensures residents / students are provided with medical care as needed. Coordinates with staffing to including referrals for outside medical treatment as needed. Document resident’s information into electronic program system. Draw labs as ordered by the doctor. Chart Check, charting labs, daily nursing notes for each resident completed daily. Weekly Nursing Assessments obtained weekly. Admission documentation completed within 24 hours. Work closely with the therapist for visitations and passes for the residents. Conducts nursing assessments on residents and accurately documents outcomes of assessments. Attends to the medical complaints and concerns by residents / students in a timely and responsive manner. Observes all restraint episodes and provides a one-hour assessment. Ensures accurate completion of incident reports and notification of incidents to Administration.
Provides medication education to residents & their families as needed. Provides notification of incidents, medical concerns, medication changes to families & referral sources.
Maintains strict confidentiality and privacy of students' and residents' personally identifiable health information in accordance with HIPAA privacy rules and Facility policies. Meets mandatory staff development requirement per the standards of all regulatory and accrediting agencies.

POSITION QUALIFICATIONS
Licensed Practical Nurse license required
Associate of Applied Science in Nursing or Bachelor of Science in Nursing, preferred, not required
Must be at least 21 years of age per Commonwealth of Virginia Interdepartmental Regulation of Children’s Residential Facilities.
Experience: Minimum of 6 months experience preferred
Licenses/Certificates:
Licensed Practical Nurse License by the Department of Health Professions in good standing.
Cardiopulmonary resuscitation (CPR)/ First Aid
Facility’s approved Behavioral Management Program
Valid Driver’s license
